Anyone in England can register with their local GP surgery.You do not need proof of address or immigration status, ID or an NHS number.
Completing a Registration Form
Our reception team can help you register with us. Please come in between 9am and 6pm and bring:
- The name and address of your previous NHS GP, or the date you have entered the country if you don’t have a previous GP.
- Details of your current and previous address.
- Photo ID and proof of address if you have these. If you don’t, we can still register you but it may take longer to trace your previous notes and/or make a referral.
- When registering infants please bring their red book.
If you do not speak English or find it hard to complete forms for any reason please let us know so that we can help with the paperwork.
Please note that your medical records may take several weeks to arrive at the Practice.
If you prefer to register without coming to the surgery please complete our registration form documented which can be downloaded on from the link below and either emailed or posted back to us.
