Please check at reception to confirm that you live within our practice area.
We will require your NHS number when you register with us. If you do not have your NHS Number, we need to ask for further details to enable our local Clinical Commissioning Group (CCG) to issue an NHS Number for you. Our reception team will be able to give you appropriate advice.
Your registration pack will include a GMS1 form, health questionnaire, online access application form and a surgery leaflet We ask that you complete the forms as fully as possible before you come for your first appointment. To complete the new patient health check you can either make an appointment with our Health Care Assistant or Practice Nurse. A new patient health check is part of the required registration procedure.
You may also be invited for regular health checks in accordance with NHS policy. If you are over 75, you have the right to be offered an annual health check and will be told who your named GP is.
Acceptance on to the list is as at the discretion of the Doctor and we will ask you for proof of identity. (Please refer to our registration policy below and why we ask for this).
Please note that your medical records may take several weeks to arrive at the Practice.
There are a number of reasons why you may not be able to register with your chosen GP, for example you may live too far away. If this is the case simply choose another GP in your local area.
