The Rapid Response Team

The Rapid Response Team (RRT) can help to avoid emergency admissions and unnecessary A&E attendances. This is a comprehensive service for all patients on the basis of clinical need.

The service can be contacted by phone or through the standard Newham referral form, however the most convenient way to contact us is via telephone on: 0208 709 5555 The service will always aim to accept any appropriate referral. If in doubt as to whether the team can support your patient – please call us. A telephone conversation will enable us to fully understand the need for support and we will always try to meet that need. The RRT is a nurse led multi-disciplinary team consisting of the following staff:

  • Community Matron
  • General practitioner (advice and consultation)
  • Consultant Geriatrician (specialist advice)
  • Nurse Practitioners
  • Rapid Response Nurses
  • Occupational Therapist
  • Physiotherapist

Conditions treated include but are not limited to

  • Chest infection,
  • Urinary Tract Infection,
  • exacerbation of long term health conditions such as COPD
  • falls
  • acute episodes of poor health in the over 60’s
  • Acopia
  • Syncope
  • Dehydration and gastroenteritis
  • Anaemia
  • Heart Failure
  • Pneumonia and Influenza

The team operates from 8am to 8pm Monday to Friday. The cut off for new referrals is 6.30pm. At weekends the team operates from 8am to 4pm and referrals will be accepted up to 2p
