Please come in between 9am and 6pm&nbsp;and bring:<\/p>\n\n\n\n<ul>\n<li>The name and address of your previous NHS GP, or the date you have entered the country if you don&#8217;t have a previous GP.<\/li>\n\n\n\n<li>Details of your current and previous address.<\/li>\n\n\n\n<li>Photo ID and proof of address if you have these.
